As you say the CCTV has been viewed and does show the room leader opening the door to the outside area at exactly the time of your son falling off his trike and going straight over to him to pick him up, along with another practitioner who was already in the playground and take him inside to be checked over. No injuries were seen by the practitioner at this time or noticed by your husband on collecting your son 20 minutes later. Both left the setting fine. In fact, your son was playing very happily shortly after the fall. Any bruising clearly came out at a later date, which can happen after a fall. We were very pleased that Ofsted looked at all the CCTV and written statements and was happy with our procedures.  We are surprised to have received 1* for every area when you have been very happy with your son being with us prior to the fall. Sadly children do have accidents and we are very pleased that he is okay. We wish you all the very best.
